Cardiac diagnostics evaluate the anatomical and physiological function of the cardiovascular system and are used when a patient presents with symptoms of conditions such as atherosclerosis, ischemia, valvular dysfunction, or arrhythmia (National Center for Biotechnology Information, 2023). Common diagnostic tools used include electrocardiograms (ECGs), echocardiograms, and CT coronary angiograms (HeartSure, 2023).

What are cardiac diagnostics?
The term 'cardiac diagnostics' refers to a variety of tests to evaluate your heart health. (Gleeson, 2020). If you have heart problems, you may be advised to have cardiac diagnostic tests.
According to NICE guidelines, patients presenting symptoms of acute coronary syndrome can undergo an initial clinical assessment to determine whether the cause is cardiac. Accurate diagnosis is determined using a combination of clinical assessments and appropriate cardiac diagnostics for further investigations. Clinicians assess patient history, the presence of cardiovascular risk factors, and any previous treatments and investigations before selecting appropriate tests (National Institute for Health and Care Excellence, 2016).
Diagnostic tests include electrocardiograms (ECG) to assess the electrical activity of the heart (Heart Rhythm London, n.d.), blood tests to measure troponin levels to identify heart muscle damage; and cardiac imaging such as CT coronary angiography, stress echocardiography, myocardial perfusion imaging, or cardiac MRIs to evaluate cardiac structure, function, and coronary blood flow (National Institute for Health and Care Excellence, 2016).
The main diagnostic tools
After a clear cardiac assessment of history and physical examinations, cardiac diagnostic tests may be ordered:
Electrocardiogram (ECG)
An electrocardiogram measures the electrical activity of your heart to detect abnormal rhythms (Heart Surgery Excellence, n.d.).
Blood Tests:
● Troponin – measures cardiac muscle damage and helps diagnose myocardial infarction (heart attack).
● Cholesterol and lipid profile – evaluates cardiovascular risk by measuring blood lipid levels, including cholesterol and triglycerides.
● BNP or NT-proBNP – measures levels of natriuretic peptides to support the diagnosis and assessment of heart failure.
Heart scans and tests
Echocardiogram (Echo)
An echocardiogram is an ultrasound of the heart. It shows how well your heart is pumping, how your heart valves are working, and whether there are any structural problems.
Stress test
A stress test shows how your heart functions under physical stress. This test can help identify problems with blood flow to the heart or abnormal heart rhythms that occur during activity.
Cardiac CT scan
A cardiac CT scan takes detailed pictures of your heart and coronary arteries. It can show narrowing or blockages caused by plaque or calcium build-up, helping to assess your risk of coronary artery disease.
Cardiac MRI
A cardiac MRI provides detailed images of your heart. It helps doctors assess the heart’s structure, function, and any damage to the heart muscle, useful for complex heart conditions.
Invasive heart tests
Coronary angiography (cardiac catheterisation)
This procedure involves inserting a thin tube (catheter) into a blood vessel and guiding it to your heart. A special dye is injected so X-rays can show how well blood is flowing through your coronary arteries. This is one of the most accurate tests for finding narrowed or blocked arteries.
Electrophysiology study (EPS)
An electrophysiology study examines your heart’s electrical system to find the cause of abnormal heart rhythms (arrhythmias). The results can help doctors decide whether treatments such as catheter ablation, a pacemaker, or an implantable defibrillator are needed (Heart Surgery Excellence, n.d.).
Why early diagnosis matters
Early diagnosis of CVDs allows early medical intervention, reducing complications and burdens of illness, improving the quality of life for patients, and reducing the socioeconomic burdens of the disease (Global Heart Hub, n.d.). Public awareness campaigns are important to educate the public about the risk factors and symptoms of CVS to ensure the individuals can monitor their cardiovascular health and know when to seek medical attention. Alongside this, primary care providers and community providers should be able to develop enhanced clinical processes to facilitate early detection and diagnosis of CVDs (Timmis et al., 2025).
